Background and Justification

In Lesotho, the prevalence of stunting remains high at 33% above the 20% WHO threshold. The high stunting is largely related to poor complementary feeding practices which continue to decline as indicated by a low Minimum Acceptable Diet (MAD) of 11% in 2014 compared to 18% in 2009 (LDHS). The 2009 food consumption study, also reported that complementary food for children is composed of a monotonous diet constituting few food groups mainly cereals, fats, some green leafy vegetables and minimal sources of animal protein. This poor diets predispose children to micronutrient deficiencies important for growth and development.

The recent available data shows that Iron Deficiency Anaemia among children under 2 years of age is very high at 51% per WHO standards with cutoff point of 20%. According to the WHO guidelines in a population where the prevalence of anaemia in children under 2 years of age is 20% or higher, Home fortification of complementary foodswith iron-containing micronutrient powders in infants and young children aged 23 months is recommended to improve iron status and reduce anaemia as well as improve the quality of complementary feeding (WHO).

Scope of Work

Purpose and scope of work

The overall objective is to design and implement a program to improve the quality of complementary feeding using home fortification with micronutrient powders (MNP) as the approach and entry point for improving complementary feeding in Lesotho.

Specific Objectives:

  • Design the implementation approach and roadmap of Home fortification with MNPs in Lesotho
  • Support the design and implement formative research to inform the implementation of the Home Fortification program
  • Facilitate the design and implementation of C4D material outlining the interventions for uptake of MNPs in the context of the broader IYCF programming
  • Develop training and capacity building package for service providers at all levels.
  • Support design and implement supply chain management and distribution system for the Home Fortification program
  • Support the design and implementation of the monitoring system
  • Support the design and implementation of the impact evaluation of the MNP programme

Approach to Technical Assistance

To provide technical assistance to the Ministry of Health Nutrition programme on point of use fortification with MNP.

Tasks

  • Design the implementation approach and roadmap of Home fortification with MNPs in Lesotho
  • Support MOH-Nutrition in the establishment of a project steering committee
  • Draft the MNP programming guideline, through a consultative process, based on global (and national IYCF guideline), as well as the available evidence.
  • Support the design and implement formative research to inform the implementation of the Home Fortification program
  • Development of a repository of communication materials on complementary feeding.
  • Facilitate the development of the terms of reference for the Formative research.
  • Support the implementation of the formative research & acceptability study which entail:
  • Study design
  • Support ethical clearance in MOH
  • Support monitoring & implementation of the formative research & acceptability study.
  • Development of MNP national brand and package design for Lesotho.
  • Support the development of MNP national brand and package design for Lesotho
  • Facilitate the design and implementation of C4D material outlining the interventions for uptake of MNPs in the context of the broader IYCF programming
  • Develop TOR for the development of C4D materials for MNP.
  • Support MOH and Micronutrient Task Team in the development of context specific C4D materials
  • Conduct pre-testing of C4D materials, revisions and finalization
  • Develop training and capacity building package for service providers at all levels.
  • Develop training materials for MNP-IYCF integration targeting master trainers, community health workers/ nutrition extension workers and caregivers
  • Conduct national level Training of Trainers on MNP-IYCF integrated package
  • Provide technical oversight for cascade of training of sub-national cadres on the MNP-IYCF integrated package
  • In collaboration with MOH and NGO partners, compile training reports
  • Conduct training impact evaluation using UNICEF guidelines
  • Support design and implement supply chain management and distribution system for the Home Fortification program
  • Guide the Supply Chain Unit of MOH and National Drug Service Organization (NDSO) on selection of product, forecasting, procurement and distribution.
  • Develop standards operating procedures for MNP distribution, coverage and reporting
  • Identify delivery channels, establish delivery platform in close collaboration with steering committee.
  • Organise training of CHW & other in the distribution & promotion of MNP in close collaboration with steering committee.
  • Support the design and implementation of the monitoring systemÂ
  • Identify key monitoring indicators
  • develop and pretest the monitoring tools.
  • preparation of monthly updates and reports
  • Support the design and implementation of the impact evaluation of the MNP programme
  • Design the pilot sites
  • Monitoring implementation of the pilot

Expected DELIVERABLES (Output level)

The consultancy will cover a period of 10 months with payments reflecting the tabulated deliverables and their respective reports, following a formal agreement and signing of the contract.

Assignment duration AND PAYMENT SCHEDULE

The consultancy will cover a period of 10 months, following a formal agreement and signing of the contract agreement, with tasks spread-out as tabulated above.

The contract will be paid upon receipt of progress report aligned to the deliverables from July 2018 to April 2019.

Supervision

The consultant will be supervised by the Chief of Section (Health and HIV) and recruited under a grade P3 per his/her level of experience. The consultant will work closely with the Ministry of Health Nutrition Programme and its Micronutrient Steering Committee.

Professional Background and Educational Qualifications

Minimum qualification and experience required to undertake the task:

Knowledge, skill and ability:

  • Managerial and Technical Experience in MNP implementation. Research skills will also be required.
  • Skills for teamwork and leadership
  • Skills to effectively monitor progress, analyze data and prepare reports
  • Computer literacy

Desired background and experience:

  • 5 or more years prior experience of working on micronutrient programmes, infant and young child feeding programme especially complementary feeding approaches
  • Excellent written and oral communication skills
  • Experience of working in the region
  • Advance university degree in nutrition or MPH with focus on Nutrition could also be added to the professional qualification

Conditions

All local travel will be reimbursed based on UNICEF policy at UN DSA rates for subsistence.

The consultant will be based in UNICEF Lesotho. Some deliverables may be completed remotely, with prior discussion and approval from the consultancy supervisor. As per UNICEF DFAM policy, payment is made against approved deliverables. No advance payment is allowed unless in exceptional circumstances against bank guarantee, subject to a maximum of 30 per cent of the total contract value in cases where advance purchases, for example for supplies or travel, may be necessary.
